Insurance

Our office will be pleased to assist in submitting your insurance claims. For us to do so in a timely manner, please present your insurance identification card at the time of your initial visit. Since you are the policy holder, you are primarily responsible for the account. The portion of the fee not covered by your insurance company should be paid on the day of surgery. We offer extended payment options with the Care Credit card. Please feel free to contact our office or visit Care Credit for more information or to apply on-line.

For your convenience, please download Patient Information forms, fill them out and bring them with you to your initial appointment. Adult dependents who are insured through a parent should fill out the Minor-Patient Information Record.
